Breast Augmentation
The primary goal, by which all operations are measured, is the satisfaction of the patient. In that regard, in that light and with that consideration, cosmetic breast augmentation produces the highest degree of patient satisfaction of all procedures in cosmetic surgery.

Patients who come to our office for consultations in breast surgery most often say. . . “I want my breasts larger.” However, what they really mean is that they wish to establish or restore BALANCE TO THEIR FIGURE. This may surprise you . . . yet it is true. Breast augmentation surgery does make your breasts larger . . . but more importantly, it restores harmony and balance to your figure. This is so important.

Scenario of Your Breast Enlargement Surgery (Breast Augmentation)
Well, that special day has arrived and now we’d like to portray for you what it’s going to be like, what you’re going to experience and how you will feel.

For clarity’s sake, let’s pick an imaginary day and time. . . let us say a Tuesday at 12:00 noon. You’ll arrive at the office, do some minimal and necessary paper work and then we’ll answer any final questions you may have. You will then be escorted to the operating room where the anesthesiologist will start an IV and you will go into a twilight state of sleep, which we promise will be very, very comfortable. The depth of this sleep will be mutually agreed upon and can be easily controlled; if you wish to be in a deep sleep that can be accomplished or if you wish to remain relatively awake, that will be fine, too. (We ask you, at this point, to refer to the Anesthesia Section of this manual.

The operation will take approximately two hours, in some cases it will be less time and in a few rare cases, a little more time. However, regardless of the duration, under anesthesia it will seem like five minutes and it will be completely painless and comfortable.

In most cases, we will employ the periareola. . . around the lower nipple approach . . . and then place the implant underneath the pectoralis major muscle. The sub muscular placement, in our view, not only produces the better result, but produces a breast that not only looks normal and natural, but feels normal and natural!

At the conclusion of the operation your bandage will be minimal, consisting of gauze and tape over the incisional area and then covered by a bra.

You will then be escorted to the recovery room approximately at 2:00 PM and you will remain there, in most cases, until approximately 4:00 PM where you will be discharged to your home. We, as well as our office staff will call you that night to see how you are doing.

The following day when you awake you will have some discomfort, but it will be closer to soreness rather than severe pain and nicely controlled by the medication we give you. You will then arrive at the office and we will remove the dressing and place on a supportive bra. We may or may not use an ace wrap on top of the bra.

On the following days we will follow you in the office and certainly by Friday you will be able to drive yourself to our complex for postoperative check-ups. We are going to ask you to refrain from rigorous exercise at this point, but certainly now by Friday you will be able to engage in normal social activities and you will be up and about.

It would be our estimate that if the operation were performed on Tuesday at 12:00 noon then the following Monday you may return to your employment. Of course, this will be modified in those women who have rather significant physical exertion at their job, but we will review that for you on an individual basis.

Between your 7th and 10th postoperative day all the sutures will be removed and incidentally, this does not hurt whatsoever. After the suture removal . . . there you are with your new augmented and enlarged breasts!

We are going to ask you to begin your exercise program approximately 14 days post operatively, but not to perform rigorous exercise for about three weeks. After that you are on your own to exercise as much as you wish.

We wish to emphasize that during this entire process we are here for you in every way: to help you, to guide you, to assist you and to give you directions. If you need us we are here and will be there during the entire postoperative period. If there will be question, if there will be concerns then we urge you to contact us without hesitation.

Scenario of Your Breast Lifting Surgery (Mastopexy)
First, let’s talk a little about the whys and wherefores of a Breast Lift or as it is known in technical term . . . a Mastopexy. It is a fact of nature that all breasts will sag to some extent, some sooner and most surely all of them later. This is due to the force of gravity combined with “tincture of time” as we say. The greater the force of gravity and the longer you are on this planet then the more tendencies for sagging. As you all know, some women start failing the “pencil test” in their mid-teens. If we add on other factors, such as pregnancy (single or multiple), fluctuations in weight, the sheering force of athletics, as well as the very common breast hypertrophy (enlargement) . . . now we can understand a little better how commonplace it is to find a multitude of women with sagging breasts.

There are, however, different degrees of sagging beginning with the most minimal cases in an average sized breast and here it can be corrected with the placement of an implant alone. For the more advanced cases, this will require an incision completely encircling the areola of the nipple along with the placement of an implant. And for the most advanced cases, we will do all that we have done above only this time, the incision will not only encircle the areola, but will extend down a vertical line to the infra-mammary crease. . .I’ll show all of this in front of a mirror.

So, there you are and that’s what we are facing if we are about to perform a mastopexy procedure. Yes, indeed it is a much more involved operation than the straightforward augmentation, but the rewards can be equally great if not greater at times!!!

So, for clarity’s sake once again, let’s go to that imaginary date and time of Tuesday at 12:00 noon at which time you will arrive at the office and then be escorted to the operating room where the anesthesiologist will start an I.V. and place you in a twilight sleep.

This time, the operation instead of taking two hours will most likely take three to four hours and once again, regardless of the duration of the procedure, it will seem like five minutes to you.

Again, the incisions we will use will either be a half moon incision on the areola for just placing an implant, a complete circular incision of the areola in the more advanced cases and a circular incision of the areola plus a vertical line down the mid portion of the breast in the most advanced cases.

Once again you will be escorted to the recovery room at the conclusion of the operation with a bandage consisting of an ace and gauze wrap, which may cover a cast-like bra. You will remain in that recovery area for approximately two hours and then you will be escorted to your car.

The degree of discomfort the following day will be no more or no less than a straightforward breast augmentation and despite the extent of this surgery; I do feel that by Friday you will be able to drive yourself to our complex for postoperative check-ups. We are going to once again ask you to refrain from rigorous exercise. Certainly by Friday or Saturday you will be able to engage in normal social activities.

If straight augmentation were performed on a Tuesday, I would advise you of your return to work on Monday. However, with a Mastopexy, I might ask you to remain home from your employment two more days. Once again, this will no way indicate incapacitation and you can work at home, use the phone, the computers and do all the things you commonly do with the exception of exercise and heavy lifting.

So, there you are! Although this is a more extensive procedure with certainly more incisional length, it imparts a wonderful degree of correction and carries a great deal of patient satisfaction.

Breast Reduction
Here at the Pavilion for Cosmetic Surgery, we have noted that many patients are dissatisfied with the scars, which are necessary in Breast Reduction surgery. This is especially true in the younger individual and we’ve also found that this scarring can be certainly independent of technique or surgical experience.

Therefore, we now employ a liposuction technique . . . with very good results . . . to reduce breast size and volume and also at the same time provide some degree of uplift. We have offered this to our patients and have found great acceptance . . . once again . . . especially in the younger patient. This is an office-based procedure and instead of a three-week recovery time as we required in Breast Reduction surgery . . . we now reduce this to but a few days.
